Vision coverage is provided through Vision Service Plan (VSP).

How the JBT Vision Plan Works

Under the JBT Vision plan, you can seek care from any licensed eye care professional. However, the plan pays a higher level of benefits when you use a provider who participates in the VSP network. Licensed eye care professionals include ophthalmologists, optometrists and dispensing opticians.

The following services are covered:

Vision exams

Once every 12 consecutive months

Eyeglass lenses

Only if needed, once every 24 consecutive months


Only if needed, once every 24 consecutive months; the plan provides a $100 allowance

Second pair of eyeglasses

You pay the retail cost, less a 20% discount

Contact lenses

VSP pays up to $100 every 24 consecutive months towards the cost of contact lenses and the associated fitting and evaluation examination

The fitting and evaluation exam is in addition to your routine vision examination and is done to ensure the proper fit of contacts

This allowance replaces the benefit for lenses and frames and applies regardless of whether the contact lenses are cosmetic or medically necessary

JBT vision coverage is designed to cover visual needs rather than cosmetic materials. Thus, limits and exclusions apply. Refer to the “JBT Vision Coverage” section of the Summary Plan Description (1.1 MB PDF) for a complete summary of what is covered and excluded under the plan.

Visiting a VSP Network Provider

You can find a VSP provider through the VSP website (www.vsp.com). Once you register on VSP, you can search for providers close to your work or home. You can also find a VSP provider by calling VSP at 1-800-877-7195.

When making your appointment with a VSP provider, inform the office you are a VSP participant. The doctor’s office will contact VSP to confirm your eligibility for benefits.

You will pay a $10 copayment to the VSP provider at the time of your visit. You are also responsible for the cost of any additional services that are not covered (e.g., the amount over your frames allowance, if any).

When you visit a VSP provider, you do not have to file claim forms; the provider takes care of this for you.

Visiting a Non-VSP Provider

If you use a non-VSP provider, you should first check with VSP to make sure you are eligible for services on your expected appointment date. Then, make your appointment with your eye doctor.

Provided you are eligible for benefits, VSP will pay the applicable allowance for your covered services. This allowance usually does not cover the full cost of treatment. To review and verify the allowances for covered procedures, visit the VSP website (www.vsp.com) or call VSP at 1-800-877-7195.

When you visit your non-VSP provider, you are required to pay the full bill at the time of service, and then submit a reimbursement claim form to VSP. You can download a copy of the claim form from VSP’s website. You can also request a copy of the claim form by calling VSP at 1-800-877-7195.

You must submit your claims for services received through a non-VSP provider within 180 days of incurring the expense. Send your claims directly to VSP at:

Using the VSP Website

You can use the VSP website (www.vsp.com) for a wide range of services. You can find a VSP provider, verify covered services and allowances, review your vision care history, and download claim forms for non-VSP provider visits.

To take advantage of these features, you will need to register through the VSP website. Simply visit the site and follow the prompts to complete your registration.
