Mental Health

JBT provides mental health coverage through Managed Health Network (MHN).

Note: If you are enrolled in the Kaiser HMO, you are not eligible for MHN mental health benefits. Instead, your mental health coverage is provided directly by Kaiser. PacifiCare HMO participants are covered under the MHN benefits described below.

How Your Mental Health Benefits Work

Mental health benefits provide coverage for the treatment of a mental disorder. The program can also provide support for coping with problems at work, school or in your home, including:

  • Balancing work and family responsibilities
  • Experiencing a loss
  • Feeling depressed
  • Dealing with stress and anxiety

If you or a covered family member needs professional assistance with a problem, you first need to contact the MHN Member Assistance Program (MAP) at 1-800-528-0646. You’ll connect with a member of MHN’s trained staff who will talk with you about the nature of your problem and a plan of action for you to consider. This may include a referral to a mental health professional for further assessment and treatment.

As with all of your health care benefits, your use of JBT mental health benefits is strictly confidential.

Visiting an MHN Provider

MHN must pre-authorize your care, and once authorized you will be referred to an MHN network provider, which includes mental health practitioners, private practice clinics, day or evening hospitals, intensive outpatient care facilities and inpatient hospitals.

Contact MHN at 1-800-528-0646 to pre-authorize your care; otherwise no benefit is payable.

Outpatient and Inpatient Mental Health Benefits

The program covers eligible outpatient and inpatient mental health services, as follows:

Up to 50 private and group counseling sessions are provided per year.

Inpatient psychiatric care and physician visits to the hospital are covered at 100%.

You pay the following copays; the plan pays the balance:

Private counseling sessions

  • 1-5 visits: $0 copay
  • 6-10 visits: $10 copay
  • 11-15 visits: $20 copay
  • 16-50 visits: $30 copay

Group counseling sessions

  • $0 copay

Services provided by non-MHN network providers are not covered, except in a life-threatening emergency.

Emergency Services

If you present an immediate danger to yourself or others and need immediate assistance, call 911 or go to the nearest emergency room. MHN’s counselors are also available 24 hours a day, 365 days a year for immediate telephone intervention.

In an emergency, pre-authorization for treatment is not required during the first 24 hours. However, to be covered for treatment after the first 24 hours, you, your provider or a family member must contact MHN immediately to evaluate the level and course of care you need. MHN may require you to transfer to an MHN network provider.

Using the MHN Website

The MHN member website (www.members.mhn.com) provides a range of emotional health and work-life resources. To use the site’s features, enter the company code: jbt.

Through the site you can:

  • View your benefits
  • Search for an MHN provider
  • Ask a mental health question
  • Find information about stress, weight management, nutrition, fitness and smoking cessation

You’ll also find tools and resources for:

  • Financial and retirement planning
  • Legal assistance
  • Finding a child care and/or eldercare provider

Information for PacifiCare HMO Participants

For diagnosis and treatment of medically necessary services for severe mental illness (for an individual of any age) or for the serious emotional disturbances of a child, your coverage will be the more favorable of:

  • The benefits as described above, or
  • The benefits provided by PacifiCare for the treatment of a physical condition, as long as that treatment is completed through an MHN network provider

The benefits for other mental disorders and for coping with life issues are as described above.
