myhealthIQ Results

About two weeks* after your myhealthIQ screening, you will receive a confidential and comprehensive health risk report. Your report includes your overall health risk score. It also provides results and information about all of the tests conducted and outlines the action steps you should consider taking to improve your health.

Your report will be mailed to your home. You can also access your confidential report online through the secure website, www.myhealthiq.com. You need to enter your user name and password to access your health risk report online. This information was included in your 2010 myhealthIQ welcome letter. If you can’t find or can’t remember your user name and password, contact Healthways customer service at 1-866-449-9705. When you visit their website, be sure to take a look at its many wellness tools and resources.

We urge you to share and review your health risk report with your doctor, especially if your results indicate you are at high or extreme risk for illness or chronic conditions. (Look for the red bars and exclamation symbols; these indicate you should consider making an appointment with your doctor as soon as possible.) If your health risk report indicates you are at high-risk for developing a lifestyle-related illness or chronic disease or having a significant medical event, you will be eligible to participate in the JBT Health Matters Lifestyle Management program at no cost. In addition, if you suffer from a condition covered through the JBT Health Matters Chronic Disease Management program, you can take advantage of that program’s support and resources to better manage your illness.

Contact Healthways customer service at 1-866-449-9705, if you have not received your health risk report. Customer service representatives are available from Monday through Friday, 9:00 a.m.–5:00 p.m., central time. You can also call the JBT Health Matters office at 1-800-284-3926.

Remember, your participation and the results from myhealthIQ are strictly confidential. Your private health information is protected by a law known as HIPAA, and will not be shared with your employer, union, physician or another family member without your written consent. If there are problems with your JBT benefits eligibility, it may take longer than two weeks for you to receive your health risk report. Every effort will be made to expedite delivery of all reports.
