Chronic Disease Management

A chronic illness or other complex health issue can take a huge toll on you and your family. Yet, if you take an active role in managing your illness, you can improve your quality of life. The JBT Health Matters Chronic Disease Management program can help, and we encourage eligible participants to take advantage of it. The program is provided at no cost to you.

How the Chronic Disease Management Program Works

The JBT works with an independent health and wellness vendor, Healthways, to provide the Chronic Disease Management program to eligible participants. The program is designed especially for individuals suffering from diabetes, heart disease, asthma, chronic obstructive pulmonary disease (COPD) or cancer. The program also covers several “impact conditions,” such as low back pain, stomach acid disorders and inflammatory bowel disorder. Refer to conditions covered through the program.

Participating in the program can help you improve the quality of your life by preventing or minimizing the effects of your disease. If you’re eligible and you choose to enroll, you’ll work with a Healthways registered nurse to learn more about your illness and actions you can take in support of your doctor’s treatment plan. An example of this would be preparing your list of questions to ask your doctor, such as what lab tests you might need; results of tests that you have had; problems with your medicines; and target goals for your treatment.

Program Eligibility

The program is available to 1400 Hour employees and their spouses, non-1400 Hour Bridged employees and Self-Pay Seasonal employees who are enrolled in the JBT Medical plan. HMO plan participants are not covered through the program—you should contact your HMO to discuss the support resources available to you.

Every month, Healthways conducts a confidential review of the JBT Medical plan benefit claims, including those for prescription drugs, doctor’s visits and hospital stays. Through this review, Healthways identifies participants with covered conditions who are likely to benefit from the program. Once identified as a program candidate, a Healthways health professional will call you to explain how the program can help you.

Your eligibility for and participation in the program is voluntary and strictly confidential.

Conditions Covered through the Program

The program covers these chronic diseases:

  • Diabetes
  • Heart disease (either cardiovascular or heart failure)
  • Asthma
  • Chronic obstructive pulmonary disease (COPD)
  • Oncology (cancer)

It also covers these conditions:

  • Atrial fibrillation
  • Hepatitis C
  • Osteoarthritis
  • Inflammatory bowel disorder
  • Irritable bowel
  • Stomach acid disorders
  • Osteoporosis
  • Urinary incontinence
  • Decubitus ulcers
  • Low back pain
  • Fibromyalgia

Enrolling in the Program

If you are invited by Healthways to enroll in the program, it’s your choice to do so. A Healthways health professional will contact you by phone to talk about your health and explain how the program might help you. If you choose to enroll in the program, you’ll begin working with a health care professional who specializes in your condition.

Here is what you can expect if you enroll:

  • One-on-one nursing support—through calls with your Healthways registered nurse or clinician who can evaluate your health status, discuss your condition and provide support references
  • Support for improving your self-care management skills between doctor’s visits
  • A health guide specific to your disease—to educate you about your disease and provide guidelines for your care, including doctor’s visits and tests; in addition, it helps you keep track of your medications
  • Newsletters—mailed to you quarterly, these provide updates on the latest treatments for your condition and reminders about how your lifestyle choices can make a difference in your health
  • Online resources—you have access to a secure, confidential website, www.iembracehealth.com

IMPORTANT: The program does not replace your doctor’s care; instead, it’s designed to support your doctor’s treatment plan and provide you with resources and support to help you help yourself. With your consent, Healthways will contact your doctor to discuss your enrollment in the program and how Healthways, your doctor and you can work together to manage your condition.

Participation Incentives

The program offers a valuable participation incentive: reduced prescription drug copayments for medications related to your condition. This incentive is offered to program participants with:

  • Diabetes
  • Heart disease (either cardiovascular or heart failure)
  • Asthma
  • Chronic obstructive pulmonary disease
  • Oncology (cancer)
  • Atrial fibrillation
  • Hepatitis C
  • Osteoarthritis
  • Inflammatory bowel disease

Your Privacy

Your eligibility for and participation in the Chronic Disease Management Program is voluntary and strictly confidential. Your private health information is protected by a law known as HIPAA, and will not be shared with your employer, union, physician or another family member without your written consent.
